Insert the brake pads and retaining clip into the caliper. Slide the brake pad pins through the caliper and the brake pads. Tighten the pins using a socket wrench.
Remove any saddlebags or whatever might be in the way as to impede your access to the rear brake caliper.
Remove the two bolts that holds the brake caliper in the caliper bracket. Set the caliper aside so that the hose is not crimped or the caliper is hanging by the hose. On a Heritage Softail with fixed calipers, rotate the caliper retaining pins until the ends of the retaining clips are expose and then remove the clips with pliers.
To replace the rotor, you will have to unbolt the rear axle on the side of the rotor. Lower the axle to make some room for the rotor to come out, then use your Allen head to remove the bolts holding the rotor in place. Be sure to achieve free play on the clutch cable to verify that you have proper adjustment on the clutch cable pack , and check for the proper specs in your service manual for adjusting the primary chain. Again, pry the brake pads apart using a putty knife. Loosen and completely remove the two pad pins.
